Your December 31 privacy program report, drafted for you.

Utah law asks the chief administrative officer of your special district or town to prepare a privacy program report every year and share it with the Utah Office of Data Privacy by December 31. The state's template runs 28 pages. You answer plain questions by email; we draft the report on that template.

You review and approve. We never send the report for you.

$199 flat for your 2026 report. We invoice after you receive your draft. Your office can pay by card, check, ACH or purchase order.Abe replies within one business day.

What the state asks for

One report a year, prepared by your chief administrative officer. Here is what the law and the template say.

  • Utah Code 63A-19-401.3 makes your chief administrative officer responsible for the report each year.
  • Since May 6, 2026, the law also requires sharing the report with the Office of Data Privacy by December 31.
  • The state's template, version 1.1, has 28 pages of questions about how your office collects, keeps, shares and protects personal information.
  • A report can say a practice is not in place yet. It also asks for your plan to improve it, so a small office can finish one honestly.

How it works

Four steps. Your part is answering questions and approving the draft.

Routing: your 2026 privacy program reportDue to the state Dec 31
  1. Write to Abe Burnett

Abe replies within one business day with the questions and a W-9 for your records. Nothing to pay yet.

    Who: YouWhen: Today
  2. Answer by email

    • Confirm the basics. We fill in your office's name, address and CAO from public records. You check them.
    • Answer in short groups. If a group doesn't apply to your office, one “no” covers it.
    • “Not sure” is a fine answer. We mark it so it gets a look before anyone approves.

We never need passwords or residents' personal information.

  3. Abe drafts your report

The state's template, filled in with your answers and read page by page against them. You get the draft within 5 business days of your answers, with a review list marked Ready, Check or Needs you. The invoice comes after the draft: card, check, ACH or purchase order.

One page of the template, before and after

Page 9 asks how your office identifies the personal data it keeps. On the left, the state's blank template. On the right, the same page from our sample report for a fictional district.

Page 9 of the state's blank template: empty checkboxes, 'Select One' in each level column and empty strategy boxes.
Blank: the state's template, version 1.1.
The same page drafted for a fictional district: Yes checked, current and target levels chosen, and a short strategy written in each box.
Drafted: our sample report, fictional data.

Every page of the sample report

Every question on the state's template has an answer in the sample. An unchecked box means the answer is no.

Each card is one page of the 28-page sample report for a fictional district. Each small square is one place on that page that takes an answer.

  • Filled in from the district's answers
  • Checkbox left unchecked: the answer is no
  • Optional text left empty, or the signature and date your CAO adds
  • Pages we point to: page 9 is the before and after above; pages 23 and 24 list who the district sells personal data to or buys it from. It does neither, so every box there is unchecked.

Three ways to get it done

The state's free template works. The question is who writes the answers.

Pricing

Two flat prices, paid once. We invoice after you receive your draft. Your office can pay by card, check, ACH or purchase order. To choose, read the three questions on the second card. If your office can answer yes to all three, the report is enough. If not, or you're not sure, take the report plus starter documents.

The 2026 report

$199

For an office that has its privacy basics in place and needs this year's report.

  • Your 2026 privacy program report, drafted on the state's own template (version 1.1)
  • Your office's name, address and CAO filled in from public records, for you to confirm
  • Abe Burnett reads every page against your answers
  • A review list: where each answer came from and what needs a look
  • The draft within 5 business days of your answers
  • Plain steps for sharing it with the Office of Data Privacy

Report plus starter documents

$499

The report asks three questions many small offices answer no. We draft what each one asks for, for your board to adopt.

  • Everything in the 2026 report
  • Has your office named a chief administrative officer? A draft board resolution that names one
  • Does your website post a privacy policy? A website privacy policy adapted from the state's template, ready to post
  • Do your employees take the state's privacy training? A training log for recording who has taken it, and when

The starter documents are drafts for your board to review and adopt. They are not legal advice; your attorney can review them.

Questions

What if we're not ready by December 31?

The report does not have to describe a finished program. The state's template lets you mark a practice as not yet in place and describe your plan to improve it.

If time is short, email us and tell us where things stand.

Is this legal advice?

No. We provide drafting help. You choose every answer and can change any of them before your chief administrative officer approves the report. For legal questions, ask your attorney.

Who approves and sends the report?

Your chief administrative officer (CAO). The law makes the CAO responsible for the report. You review and approve. We never send the report for you. Your CAO approves the final report before it is shared.

Where does the report go?

Page 2 of the state's template says to email it to the Office of Data Privacy. The Office's website says sharing instructions are coming. We show your CAO both and check the website again before your report is due. Your CAO sends it.

Can we pay by invoice or purchase order?

Yes. We invoice after you receive your draft. Your office can pay it by card, check, ACH or purchase order. Need a quote for your board first? Ask for a board packet: a one-page quote and our W-9.
